When completing the nomination form, it is important that you respond to every question and provide as much information as possible to demonstrate that the contributions of the individual merits consideration by the Awards & Recognition Committee.
Please note that the criteria, in an un-weighted fashion, will be used to determine the Hall of Fame designation. Because the official criteria for the designation is somewhat general in nature and subject to varying interpretation, the following considerations are viewed as reflecting appropriate guidelines for bestowment of this most prestigious award:
The nomination form is confidential. Nominations must be received by Feb. 8, 2013 to the attention of Linda Webb-Manon, TCUL vice president of communications & public relations, at firstname.lastname@example.org, or via fax (469) 385-6586. Up to two individuals may be selected from the pool of nominees. Please note that if you submitted a nomination in 2011 and that individual was not chosen, it is not necessary to resubmit, unless you wish to provide updated information. If you submitted a form prior to 2012, you will need to resubmit.
